1. Review and sign Venue Agreement

2. Complete the Practice Information Form.

3. Verify DSL service is installed and ongoing in your practice.

4. Review the Customer Care Package. Attach your check for the appropriate amount.

5. Fill in your practice name and fax Insurance request to your insurance agent.

6. Return Practice Information Form and signed Venue Agreement with your check or Credit Card Authorization to:
